What happens if the wrong player goes first in American Mahjong?
If someone other than East discards first, they take the tile back and East throws, with no penalty. If someone other than East draws from the wall and discards first, the hand is thrown in and dealt again. If the wrong player has been sitting as East and nobody notices until play has begun, that hand is played out and the next one goes back to the rightful East.

The game begins with East
Every hand opens the same way. After the Charleston, East holds fourteen tiles and everyone else holds thirteen. East does not draw. East simply discards, and that discard is the moment the game begins.
So the first tile thrown belongs to East and to nobody else. Until East has thrown it, the hand has not started, and that is why the early mistakes are treated so gently.
Someone else throws first
An eager player tosses a tile into the center before East has moved. It happens at every table, usually to someone who has just finished a very satisfying Charleston.
The remedy is simple. The player takes the tile back into their hand, East makes the opening discard, and play goes on to the right as usual. There is no penalty, because the game had not yet begun when the tile was thrown.
Someone else draws and throws first
This one is more serious. If a player other than East draws a tile from the wall and then discards before East has opened, the hand cannot be put right. A wall tile has left its place and been seen. The tiles are thrown in, washed and dealt again.
There is one small mercy on the first round of play. Once East has opened, a player who discards before drawing on their very first turn may take that discard back. Later in the hand the same slip leaves twelve tiles on the rack and a dead hand, so enjoy the forgiveness while it lasts.
The wrong person is sitting as East
Sometimes the seats themselves are muddled. The dice were rolled and misread, or the East seat passed the wrong way after the last hand. If nobody notices until the hand has begun, that hand is played out as it stands. Nobody unpicks a game in progress.
The next hand goes back to the rightful East, and the round carries on from there. The player who took East in error is skipped when their own turn as East comes around, so nobody gets an extra helping.
How to keep it from happening
East is the player who broke the wall and took the first four tiles of the deal, and East is the only player holding fourteen tiles after the Charleston. Count your tiles before and after the passing, as you should anyway. If you hold thirteen, you are not throwing first. Wait for East, and let East have the opening line.
